First Site Evaluation

Start with the question.
Keep the records private.

Start with the decision and the kinds of records available, not the records themselves. This form prepares an email draft for the initial fit discussion; it does not send a message.

Site type starts the fit discussion; it does not establish eligibility. The equipment arrangement, question and available records determine the supported scope.

Do not paste sensitive records, personal data beyond your contact details, access credentials, or confidential site information. Scope and data handling are agreed before records are shared.

You send the draft from your email app. This page does not submit a message.

A useful introduction

What would you like to establish?

  • The site and proposed additional load.
  • The period you want to examine.
  • The categories of records available.
  • Any known gaps or questions to resolve during scoping.

The First Site Evaluation is quoted in writing before work begins, subject to suitable records and an agreed supported question. Written authorization, recipients and data handling are agreed before records are shared.

Written finding within five business days after scope is agreed and the required records are confirmed.
